Paws Glorious Paws - Teeth Cleaning Consent Form 

I confirm - 

  • My pet is fit and well with no known serious dental issues 

  • I am happy for Vicky at Paws Glorious Paws to carry out teeth cleaning on my dogs’ teeth using their equipment which includes a ultrasound toothbrush, toothpaste and hand tools. 

  • You are happy for before and after photos be taken. These can be shared with you if requested along with other customers and social media. 

I understand that - 

  • I understand this is teeth cleaning and not a dental treatment 

  • Paws Glorious Paws does not use any drugs or sedatives 

  • The teeth cleaning is like that of a human hygienist 

  • The teeth cleaning may uncover issues that may require an appointment with a vet. 

  • Paws Glorious Paws cannot diagnose medical issues 

  • Your pet can eat and drink normally before and after the cleaning

  • Some redness of the gums and minor bleeding is to be expected. This will stop quickly and cause no aftereffects. 

  • If an appointment needs to be cancelled or moved, we require 24 hours’ notice. If missed or moved with less notice we reserve the right to charge a cancellation fee which will be 50% of the cost of the appointment. 

Paws Glorious Paws will - 

  • Stop the teeth cleaning is your pet is unhappy or aggressive. 

  • We will tell you on collection if there have been any issues or we have found any problems with your pet 

  • We will discuss with you the number of treatments needed. It may be more than one session. 

  • We recommend at least 4 sessions a year to keep their teeth & gums in good conditions after the initial treatment. These are usually 15-20 minutes long and can be done at the same time as a groom or as a separate appointment. 

I declare that: I do not know of any medical reason why teeth cleaning cannot be carried out on my dog and I acknowledge that it is my responsibility to inform Vicky in the future should this change. 

I am aware that the teeth cleaning service is not a substitute for a veterinary inspection and Vicky is not qualified to diagnose abnormalities or disease. 

I consent to all the above and confirm that I am the legal owner/guardian of this dog.
